5/2/2023 EnchantaVaughn Approved (AM)Client was having a tough morning as soon as he entered the building. Client was hyper aggressive and continously shouted inappropriate statements to his peers in the hallway; symptoms which directly correlate to his current diagnosis of Attention Deficit Hyperactive Disorder. Client required to be redirected 3-4 times before he followed through with authority directives and prompts. Client had a hard time managing his emotions and staying on task. Client often acts out and ignores instructions,, displaying low responsible behaviors at camp. Client is constantly prompted to control his temper and calm down. Client was warned that if he continues to misbehave he will be written up. QMHP was asked to speak with him per request of his homeroom teacher. Teacher reported client was off when he walked in the class and stormed to his seat. Client did not participate in the morning assignment nor listen attentively to the announcements. Client had an attitude until counselor arrived. (PM)Client was inquired why he was upset. Client shrugged his shoulders and continued inappropriate behaviors by smacking his lips and rolling his eyes at QMHP. Client was given quiet time to get his thoughts together and reflect on his behaviors. Client finally calmed down and explained why he was mad. Client stated that people was teasing him and saying mean things. Client was reminded to tell his teacher and stop allowing people to mess his day up. Client promised to turn his day around. Client was encouraged to follow directions and engage fully in his exercises. Client was reminded that he has been improving a lot and need to make better choices moving forward. Client was reminded that school is about learning together and being able to interact with our peers on our age level so that we learn the fundamental principles and develop a sense of maturity and self-control. Client appeared to understand everything that he was being told and nodded his head in agreement.(EOD) Client was seen at his seat in a normal mood and supported with being back in class. Client got a little bit of work finished prior to preparing for the end of the day. Client packed his belongings then left school without any incidents and is expected to have a better day tomorrow.
